Loudoun Medical Group Earns National Distinction As Patient-Centered-Medical Home (PCMH) For Primary Care Practices By The National Committee For Quality Assurance (NCQA)

LEESBURG, Va. (September 15, 2023) Loudoun Medical Group (LMG), the leading multi-state, multi-specialty group practice has been awarded recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program. Recognized LMG practices include; Ashburn Pediatrics, Kingstowne Internal Medicine,Loudoun Valley Family Practice, Mitchell Whitaker & Wu, MountainView Medical Associates, Loudoun Internal Medicine Associates, Loudoun Pediatric Associates, Leesburg Sterling Family Practice, Pediatric Healthcare and Potomac Family Practice. The mission of the National Committee for Quality Assurance (NCQA) is to reduce healthcare costs while improving quality across key domains.

NCQA’s Patient-Centered Medical Home Recognition program is the most widely adopted PCMH evaluation program in the country. More than 10,000 practices (with 50,000+ clinicians are recognized by NCQA. Many payers acknowledge PCMH Recognition as a hallmark of high-quality care. The NCQA PCMH program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others.

“In recent years, there has been a shift towards value-based care,” said Clara McAuley Nussbaum, MPH, Director of Value Based Care, Loudoun Medical Group. “Annually, we recertify that our primary care practices are able to meet rigorous industry standards and best practices for assessing population needs and stratifying populations by risk so early interventions can be implemented.” Organizations that want to succeed under value-based arrangements must implement effective population health management to improve patient experience, lower healthcare costs, and maintain a high quality of care.

A key element of the PCMH model is engaging patients and caregivers in their care. Interoperability and the expansion of Health IT provides important opportunities to harness capabilities to support and encourage patient engagement. Loudoun Medical Group has joined the National Interoperability Initiative to improve patient care coordination and enhance health outcomes with PRISMA Interoperability Search Engine.
