Medical Home Responsibilities 

What Is a Medical Home?

The American Academy of Family Physicians defines a medical home as one that is based on the Joint Principles of the Patient-Centered Medical Home (PCMH), the Shared Principles of Primary Care, and the five key functions of advanced primary care. Through implementing medical home functions, you can improve the quality, effectiveness, and efficiency of the care you deliver while responding to each patient’s unique needs and preferences.

Five Key Functions of a Medical Home

No matter where you fall on the spectrum of practice improvement—managing current projects, enhancing basic concepts, or advancing to more complex initiatives—adopting the five key functions of a medical home can benefit your practice, your patients, and your bottom line.

  • Access and Continuity - Provide the care your patients need—anytime, anywhere.
  • Planned Care and Population Health - Use technology to help you be proactive about managing patients’ chronic conditions.
  • Care Management - Identify which patients need additional support from your care team and provide those services.
  • Patient and Caregiver Engagement - Encourage patient participation and shared decision making.
  • Comprehensiveness and Coordination - Build relationships and coordinate care across the medical neighborhood.

Your Medical Home Team consists of your primary care practice as part of Loudoun Medical  Group, P.C. (LMG).

What services does my Medical Home provide?

Patients, families, and caregivers can expect to receive evidence-based care from their Medical Home Team. The Practice provides educational resources for specific health issues to support self-management of your health and healthcare.

It is the responsibility of the Medical Home to provide whole-person care. This means coordinating your care across multiple settings, including visits and referrals to specialists, behavioral health professionals, the hospital, and other facilities. Behavioral health needs are addressed by the Practice and in coordination with behavioral health professionals, as needed.

Your LMG primary care practice functions most effectively as a Medical Home if patients and families provide a complete medical history and information about care obtained outside the Practice.

Additionally, the Practice requests that patients provide their LMG provider’s information to other providers/facilities outside the Practice.

How will I contact my Medical Home Team?

When new patients are oriented to the Practice, staff explain the roles of the care team and who to contact for specific concerns or questions during and after office hours. More specifically:

Please contact your Medical Home Team directly to arrange an appointment or to discuss your medical needs. Please visit https://www.lmgdoctors.com/ and click on the ‘Find a Provider’ link to locate your practice, make an appt, see location hours and services. Patient calls for routine clinical advice will be returned within two business days or within one business day for urgent clinical advice.

For routine or urgent matters outside of the normal office hours, please contact LMG urgent care practices in Sterling (703) 450-1125 or Leesburg (571) 707-2085 for in person or virtual care.
